What to bring to your appointment

1. Bring any referal letters that you may have.
2. Bring any relevent X-Rays or scans or reports.
3. Wear suitable clothing to allow for examination of the area involved.
4. Remember your private health insurance card, DVA gold card, medicare card.

• Treatments must be paid for at the time of consultation.
• Please try to arrive 10 minutes before your initial consultation to fill out the necessary paperwork.
• Please try to be on time. We aim to be punctual 98% of the time.
• If running late, a phone call would be great.

Workers Compensation and CTP Claims

• Bring your insurance details to your initial consultation.
• This will include a claim number and insurance detail, and enables us to bill the insurer directly.
